Client Rights & Access to Records

Texas Health & Safety Code §181.105 (HB 4224)

Your care, dignity, and access to your information matter deeply here.

In accordance with Texas law, you have the right to request access to your health care records and to receive information about how to contact the appropriate licensing authority and file a complaint.


How to Request Your Records

You may request a copy of your records at any time.

To do so, please:

  • Submit a written request via your secure client portal 


If you no longer have access to the client portal, please follow the steps below:


Step 1: Send an Initial Email Request

Please send a brief email requesting records to:
Rosemary@WideAwakeTherapy.com


To protect your privacy, do not include sensitive personal or health information in this initial email (such as date of birth, clinical details, or Social Security number).


You may simply state your name and that you are requesting your records.


Step 2: Secure Verification Process

After your request is received, you will be guided through a secure process to:

  • Verify your identity 
  • Specify the records being requested 
  • Sign any required authorization forms 


This step helps ensure your information is released safely and in accordance with HIPAA and Texas law.


Step 3: Record Delivery


Once verification is complete, your records will be provided through a secure method.
Reasonable fees may apply as permitted by law.

NOTICE TO CLIENTS

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology.


Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint. Please call 1-800-821-3205 for more information
